Once you notify your manager and HR Business Partner of your need for time off for a reason related to STD or FMLA, for any absences exceeding 5 consecutive days, you may be required to file for STD and/or FMLA.
If your claim decision is Pending with Unum after any applicable waiting period has ended (or for a subsequent extension request) you may not receive pay while you are out of work until your claim has been approved. Once approved, you will receive any retroactive pay due, if applicable, on your next scheduled paycheck.
Yes, time away from work may impact Variable Compensation (VC) and Sales Incentive (SI), if eligible. Your bonus payment will be subject to proration if your leave of absence exceeds 30 calendar days.
If you are eligible for a Merit Increase while on leave, the increase will be postponed until you return to work/active payroll status. The effective date of the increase is the date of your return.
The HR Benefits Team will enter any applicable codes to an Associate’s timecard while on leave. This includes vacation and/or paid sick leave during the waiting period (if applicable) as well as during the remainder of the Associate’s leave. Leave codes will be entered to an Associate’s timecard as the time is approved by Unum.
If a company holiday occurs while you are on leave, that day is considered part of your leave and will be paid as either Short Term Disability or Paid Parental Leave. If a holiday occurs while you are completing your one-week waiting period, that day is considered a part of your waiting period and available Vacation/Paid Sick Leave will apply. Holidays do not generally extend the total duration of your leave.
Vacation accrual continues while on a paid leave. You will not accrue sick or vacation time during an unpaid leave.

You have 30 days from the date of birth/adoption to add your new child to your benefit plans via Self-Service. Select Report a Life Change and choose "Add New Child." When adding your new dependent, you will be asked for the SSN. If you do not have it yet, check the box indicating that the SSN is “applied for.”

Your benefits will continue while you are on leave; however, you are responsible for your portion of the premiums for the plans in which you are enrolled.

If you are enrolled in Basic Associate Life, Basic Associate AD&D and Supplemental Associate Life you may want to update beneficiary designations for each plan accordingly. To update your beneficiaries for your 401(k), log into your Fidelity account at netbenefits.com.
Some states provide leave benefits that you may be entitled to. When you file a leave claim, Unum will let you know if you are eligible for any state benefits. If applicable, you’ll need to file a claim directly with your state. Company-provided leave benefits are generally offset by statutory benefits and run concurrently. You should review the Leaves of Absence and Other Time Off Policy for additional information.
The Family Medical Leave Act (FMLA) provides up to 12 weeks of unpaid, job-protected leave for a variety of circumstances. You’re eligible for FMLA if you have worked for at least 12 months and have worked 1,250 hours during the previous 12-month period. FMLA is taken concurrently with other types of leave. If you have exhausted FMLA or are not yet eligible for FMLA, you may be eligible for job protection through Supplemental Medical Leave.
